That ramp was wide, dry, and had a shallow angle of climb, so easy to load that bike.

I'll bet what happened was he was real nervous about it thinking about what could happen, and just as the way you tend to hit a rock in the road that you focus on even tho U R trying to miss it, same kind of thing happened to him.
That's called "Target Fixation", never look at the pothole in the middle of a corner, just notice it and then look at the place you intend riding to.

"Target Fixation" is well understood, in fact it's one of many points covered beginner rider classes.
I've seen so many vids of people riding into banks and fencing on corners at reasonably slow speed because they are looking at the fence earth bank etc instead of looking further up the road.
Yes I have actually watched as rider have done this but for some reason I have never had any issues with this happening to me. Maybe it is because when I see danger I immediately start looking for the escape route so my attention is taken away from focusing on the actual issue at hand.
