I often ride alone in sometimes remote coastal areas so I always carry plenty of personal ID. Better to be prepared than not.
A few months ago I had this dog tag made with the same personal info. It's flipped over to protect my ID, but I assure you, it's there
It carries all the info a first responder would need to contact relatives and details about my physical health.
Knock on wood, I don't have any physical or medical issues.

Yes, they seem to have something for everyone.
I also have one of their wrist bands.

I just use my military dog tags and I carry good, solid ID's and my phone shows my ICE contacts.
